Your First Appointment
Here’s what you need to bring with you:
- • Your test results and the referral form from your primary care physician
- • Your fully-completed patient forms.
For Bariatric Surgery or Nutrition Patients
Please download our new patient bariatric forms here.
For General Surgery Patients
Download our new patient general surgery packet.
For Vascular Surgery Patients
Please download a new patient vascular surgery packet.
Once you have downloaded the forms, you can print them, complete them and bring them with you for your first appointment.
During this appointment, one of our surgical specialists will go over your medical history with you and perform a physical exam. After the surgeon reviews these medical records and diagnostic test results, additional tests may be ordered.
Once your testing and evaluation is complete, the surgeon will talk to you about the recommended treatment plan and answer your questions. If a surgical procedure is recommended, we will fully explain it and also schedule the procedure for you at a hospital or in our state-of-the-art outpatient surgery center.
In general, patients are advised not to eat or drink anything after midnight the day before surgery. Specific recommendations about anesthesia in your case will depend on the type of surgery you will be having and your current medical condition. We will go over all of this information with you and give you a written checklist to remind you what you can and can’t do before your surgery. If you have any questions before your procedure, please call us.
What happens immediately after your surgery depends on the type of procedure you had and the general state of your health. We will go over all the details with you and your family members to help you understand what to expect after surgery and what type of post-surgical care our medical team will provide. If you have any questions after your procedure, please call us.
